TYPE 2 DIABETES A POLYGENIC FORM OF DIABETES Type 2 diabetes mellitus (T2DM) arises out of a combination of insulin resistance and insulin secretory defects. This interaction is, in turn, influenced by environmental factors such as diet, obesity, and physical activity. T2DM is known to have an even stronger genetic predisposition than T1DM, with a high concordance rate among identical twins (approx 80 as opposed to dizygotic twins, for whom the risk is approx 10 ) and an increased incidence in first-degree relatives (approx 10 ) (134,135). Genome scanning approaches have shown linkages to a large number of chromosomal locations (136), which vary among populations. For only one of these, 2q37 in Mexican-Americans, has a candidate gene been localized and shown to associate with diabetes.

A misshapen body habitus, such as that caused by scoliosis or, more commonly, morbid obesity, may make SWL unfeasible because the patients cannot be positioned so that the stone is in the focal point of the machine. PCNL may also be technically demanding in these patients but carries fewer risks than open surgery does. A preprocedural CT scan is helpful in such patients to evaluate the anatomy and plan an access route that avoids bowel and other viscera (238). PCNL has also been successfully applied in the treatment of calculus disease in anomalous kidneys, such as crossed-fused renal ectopia, horseshoe kidneys, and pelvic kidneys. These complicated cases also call for extensive preprocedural planning.

Spoilage Of Fresh Mushrooms

Postharvest storage conditions significantly contribute to mushroom quality and shelf life. Pai 53 evaluated the effect of storage temperature (5, 10, and 15 C), and relative humidity (RH) (91, 94, 97, and 99 ) on weight loss, whiteness change, and microbial activity of A. bisporus mushrooms. Weight loss of tested samples was correlated highly with storage time at each RH level. Increasing storage temperature and decreasing RH significantly enhanced (p 0.05) the rate of weight loss. Mushroom whiteness values were not affected (p 0.05) by changes in RH. Microbial growth increased with increasing storage temperatures. It was concluded that the use of clean mushrooms with low initial microbial counts, an environment of high RH, and Overwrapping mushrooms with plastic film improves their quality as observed by rate of cap opening, color, and weight loss 56-58 .

Microvascular Permeability in Preeclampsia

Leptin, a protein product of the obesity gene, plays an important role in regulation of body weight in the nonpreg-nant state through its receptors in the satiety center of the hypothalamus. Endothelial cells also express leptin receptors. Their activation induces angiogenesis and increases microvascular permeability. It has been suggested that the increased circulating leptin found during pregnancy is of placental origin. Although the functions of leptin in the placenta are not known, in addition to its angiogenic effect it has been suggested that may help to increase the exchange of small molecules between the maternal circulation and the fetus by enhancing convective mechanisms and by the induction and maintenance of increased vascular permeability. Placental expression and circulating levels of leptin are significantly increased in pregnancies complicated by pre-eclampsia. structurally, leptin has been shown to resemble class I cytokines.

Metabolic Idiosyncrasy

Although it was initially thought that the toxicity of reactive metabolites only caused cell necrosis, this idea has been challenged in recent years (Pessayre et al., 1999). It is now clear that the extensive formation of reactive metabolites can cause apoptosis, necrosis or both (Fau et al., 1997 Shi et al., 1998). Several compounds, such as acetaminophen and cocaine, transformed into reactive metabolites have been shown to cause DNA fragmentation of hepatocytes indicative of apoptosis (Shen et al., 1992 Cascales et al., 1994). The cellular mechanisms causing metabolite-induced apoptosis have been studied with germander, a medicinal plant used in weight control diets, the widespread use of which led to an epidemic of hepatitis in France (Larrey et al., 1992b). Germander contains furano diterpenoids, which are activated by CYP 3A into electrophilic metabolites (Lekehal et al., 1996).

Pharmacologic options

Patients with a BMI 30kg m2 or with a BMI 27kg m2 with concomitant obesity-related diseases should be considered for adjuvant pharmacologic therapy. Pharmacotherapy is appropriate in individuals who have not responded to previous weight loss attempts (diet, exercise and behavioral changes) or have been unsuccessful with sustaining previous weight loss attempts. The two medications approved by the FDA for long-term weight loss are sibutramine (Abbott laboratories, Abbott Park, IL a norepinephrine and serotonin reuptake inhibitor that enhances satiety) and orlistat (Roche Pharmaceuticals, Nutley, NJ a lipase inhibitor that reduces fat absorption). Both agents are approved for long-term use, have proven effective in large-scale multicenter trials and have side effects that may be easily monitored by the clinician. Sibutramine produces a dose-dependent weight loss (available doses are 5, 10 and 15 mg capsules), with an average loss of about 8-10 of initial body weight at 6 months 28 .

Diagnosis and classification

The regular use of extreme methods of weight control (e.g. highly restrictive dieting, self-induced vomiting, the misuse of laxatives and diuretics, or overexercising). expressed as an intense dissatisfaction with shape and weight, a fear of weight gain and fatness, and, in many cases, a pursuit of weight loss and thinness. In addition to the three required diagnostic features, there is an exclusionary criterion. This is that the person must not meet diagnostic criteria for anorexia nervosa. In practice, this means that the patient must not fulfil the low-weight criterion for anorexia nervosa (a body mass index below 17.5). The principal diagnostic criteria for anorexia nervosa and bulimia nervosa are summarized in Table 1.

In many laboratories planar imaging is currently only performed in overweight patients who are too heavy for SPECT imaging tables. In these obese patients, it is our policy to be conservative in interpreting planar images in the obese. Only unequivocally abnormal features are reported as abnormal. More subtle inhomogeneities are presumed to be equivocal or due to attenuation.

One reason for this is that LC-SFAs require carnitine acyltransferase for transport into the mitochondria for oxidation,25 so they are, therefore, more readily incorporated into TAGs. The MuFA are readily utilized for energy by P-oxidation in mitochondria but may also be stored in adipocyte TAGs.27 The long-chain rn-3 and m-6 PuFAs ( 18 carbon) (Figure 3.4) are a significant component of cellular phospholipids but may also occur, to some degree, in TAGs.

As indicated above, proteins in the body are not static. Just as every protein is synthesized, it is also degraded. Schoenheimer and Rittenberg first applied isotopically labeled tracers to the study of amino acid metabolism and protein turnover in the 1930s and first suggested that proteins are continually made and degraded in the body at different rates. We now know that the rate of turnover of proteins varies widely and that the rate of turnover of individual proteins tends to follow their function in the body, i.e., proteins whose concentrations must be regulated (e.g., enzymes) or that act as signals (e.g., peptide hormones) have relatively high rates of synthesis and degradation as a means of regulating concentrations. On the other hand, structural proteins such as collagen and myofibrillar proteins or secreted plasma proteins have relatively long lifetimes. However, there must be an overall balance between synthesis and breakdown of proteins.

A patient with bronchogenic carcinoma may report symptoms including cough, sputum production, or hemoptysis at first presentation for evaluation by a physician. Other patients have advanced disease and nonspecific systemic symptoms, including weight loss and weakness, at initial examination. Chest radiographs may show findings that suggest lung cancer. Still other patients report no symptoms related to lung cancer, and abnormal results on chest radiographs prompt further workup. In almost 40 of patients with lung cancer, the disease has spread outside the thorax.1

Overview of nutrition and lipogenesis

Lipogenesis

Lipogenesis encompasses the processes of fatty acid synthesis and subsequent triacylglycerol synthesis, mainly from excess carbohydrate in the diet. The main sites of lipogenesis are liver and adipose tissue. A detailed overview of lipogenesis and other processes of lipid metabolism is presented in Chapter 1 and here only some specific aspects will be addressed. The traditional view is that low activity of the enzyme glycerol kinase in adipocytes ensures that triacylglycerol formation in these cells is dependent on an optimal supply of glucose, avoiding re-esterification under conditions in which fatty acids are to be exported, such as fasting. However, cycles of triacylglycerol hydrolysis and re-esterification do occur within the adipo-cytes, even during fasting, because glycerol-3-phosphate can be produced from precursors (other than glucose or glycerol) through glyceroneogene-sis, which is an abbreviated version of gluconeogenesis that occurs both in liver and adipose tissue.

That the ALF provided opportunities for companionship was more than lost on Doreen she ignored or avoided these efforts. During the first several weeks, the dining room staff tried, as they did for all new residents, to introduce her and seat her with others at meals, observing which pairings or groupings appeared to work best. Doreen passively accepted being seated anywhere, with anyone. She voiced no complaints. Other residents, however, did have complaints. Her eating behavior was most unattractive. The biology of NF-kB in endothelial cells, and its possible modification by dietary components, has attracted considerable attention because of accumulating evidence that this transcription factor might play a major role in atherogenesis. First, activated NF-kB is present in the atheromatous plaque in SMC and macrophages as well as endothelial cells but is absent in cells of the healthy arterial wall (8). Second, NF-kB is activated by an array of stimuli thought to play a key role in the development of atherosclerotic lesions, including tumor necrosis factor alpha (TNF-a), interleukin-1 (IL-1), bacterial lipopolysaccharide (LPS), advanced glycation end products (AGEs), hyperglycemia, platelet activating factor, shear stress, oxidized lipids, oxidant stress, and hypoxia reperfusion (9).

Major depression, however, is a complex disorder that can be divided into two major subtypes melancholic and atypical. The features of melancholic depression include insomnia (most often early morning awakening), loss of appetite, weight loss, accompanied with negative behavioral changes, while the second major subtype is major depression with mostly opposite, atypical features characterized partly by hypersomnia, hyperphagia, lethargy, and fatigue.

The components of energy expenditure

The total expenditure of energy over a 24-hour period can be broken down into the basal metabolic rate, the energy cost of physical activity and diet-induced thermogenesis (Fig. 11.3). Physical activity varies considerably from person to person. However, the largest component of the 24-hour energy expenditure is, for most people, the basal component. The basal metabolic rate is very closely related to the amount of non-fat tissue in the body, the fat-free mass or lean body mass.2 The larger someone's fat free mass, the larger (in general) their basal metabolic rate (this will be illustrated later, in Fig. 11.5). The basal metabolic rate is also regulated by hormones, primarily by the thyroid hormone, triiodothyronine. During starvation or food deprivation thyroid hormone concentrations fall and basal metabolic rate decreases (see Section 7.3.2.1). The significance of this for weight reduction programmes will be discussed again later.

Sources and properties of nondigestible oligosaccharides

Several NDOs are considered DFs, itself a broad category. As illustrated in Table 7.1, NDOs may occur naturally in many plants - mainly vegetables, whole grains and fruits (Meyer 2004). Another natural source of NDOs is milk (cow milk galacto-oligosaccharides and human milk oligosaccharides). Moreover, several NDOs - often added in food for their technological properties - may be synthesised from simple or complex carbohydrates. NDOs present in the diet differ from one another in their chemical structure, in other words the number (DP) or the type of hexose moeties (glucosyl-, fructosyl-, galactosyl-, xylosyl-), the position of links between the hexose moeties and their conformation (a- versus P-) (Delzenne 2003). All these characteristics have consequences on the physical properties of NDOs - and therefore on their putative usefulness as food ingredients and their effects and metabolism in the gastro-intestinal tract.

Physiological factors influencing food intake

Long-term food intake regulation is essential in food-weight management. The hormone leptin appears to be involved in long-term food intake regulation. Leptin is synthesized mainly by adipose tissue it acts through receptors present in afferent visceral nerves and the hypothalamic arcuate The balance and interaction between anorexigenic (e.g. CCK, PYY) and orexigenic (ghrelin) factors originating from the gastrointestinal tract appear to play an important role in short-term regulation of food intake. An impairment of this balance may result in disorders of feeding behavior and weight gain (obesity) or weight loss (cachexia). Understanding this balance is essential in developing foods that help people maintain a healthy body weight.

Intestinal iron transport

Our current understanding of intestinal non-heme iron transport is illustrated in Figure 13.1. Iron absorption takes place in an acidic environment in the proximal small intestine, just distal to the gastric outlet. Most non-heme dietary iron is in the ferric (Fe3 +) form. It is reduced to Fe2 + by a brush border ferrireductase, most likely the recently identified duo- The cartoon shows an absorptive enterocyte from the duodenal epithelium, joined to adjacent cells by iron-impermeable tight junctions. The apical brush border is at the top and the basolateral surface is at the bottom. Dietary Fe3 + iron is probably reduced by the ferrireductase DCYTB to produce Fe2 + ion for transport. Fe2 + crosses the apical membrane through the action of DMT1 to enter the cell. There, iron is partitioned between storage and export stored iron is ultimately lost from the body when the epithelial cells senesce and exfoliate into the gut lumen.

Restrictive Lung Disease

Restrictive lung disease is another category of pulmonary disorders that may require nocturnal PAP therapy. Lung restriction is most commonly seen in obesity, kypho-scoliosis, neuromuscular disease, interstitial lung disease, and pregnancy. PAP therapy should be used unequivocally in these patients if they have coexisting OSA. However, in the absence of OSA, PAP therapy must be considered on a case-by-case manner, depending upon the disease process being considered. Obese patients without OSA but with obesity hypoventilation syndrome (OHS) may benefit from nocturnal PAP therapy CPAP, BPAP, or volume cycled NIPPV. BPAP or volume cycled NIPPV allow for increased ventilatory assistance compared to PAP therapy with CPAP. Initiation of PAP therapy should be performed

Starch composition and arrangement

Both starch polymers can theoretically be enzymatically digested within the small intestine. However starches with elevated levels of amylose tend to have higher inherent resistance to amylolysis, particularly in their granular form. The most researched and commercially used form of RS is obtained from high-amylose maize or corn and it is this material that has been used in experiments to investigate the potential impact of RS in weight management regimes. digested by amylases (Brown et al., 2003). In contrast, high-amylose corn starch, as a result of its granule structure and conformation, resists gelati-nization during most normal food processing conditions, such as in the baking of bread, and contributes RS and dietary fiber to the food.

Adverse Reactions to Insulin Therapy

Another frequent side effect of insulin therapy is weight gain. Some is due to increased caloric storage of glucose by insulin, and some is due to renal sodium retention resulting in fluid retention and edema. These effects can synergize with oral agents that are often coad-ministered with insulin, particularly sulfonylureas and thiazolidinediones.

The expression of these transporters is regulated by the availability of dietary carbohydrate. SGLT-1 expression is increased by glucose availability in the small intestinal lumen (see Table 2.6). Fructose availability is a specific signal increasing expression of GLUT5. From within the mucosal cells, the sugars enter the capillaries that form a dense network within each villus (see Fig. 3.3). They must first cross the cell membrane at the 'back end' of the cell - the basolateral membrane (Fig. 3.7), to enter first the interstitial space and then the blood in the vessels draining the small intestine towards the portal vein. This basolateral transport is by facilitated diffusion mediated by GLUT2, which will transport both glucose and fructose. Active transport of sugar into the cell from the intestinal lumen must raise its intracellular concentration to the extent that it will move out, into the interstitial space, down a concentration gradient.

(2) Measurement of dietary antioxidants in vivo. These methods generally measure the concentration in plasma, whole blood, or tissues, of enzymes that contribute to the antioxidant defense or of several antioxidants. Such methods include measurement of enzymes such as superoxide dismutase (SOD), catalase, and glutathione reductase (GTr) (95). Other methods include the determination of tocopherols, vitamin C, carotenoids, some polyphenols, uric acid, and others (96,97).

Lenore became very jealous of other residents who, she perceived, got more attention, better service, or more food than she did. In time, this jealousy escalated to rage and acting out. Lenore began to take food from these residents' plates, resulting in her being banned from the dining room for the next meal or the next day. Soon she was eating most of her meals in her room. This made her even more angry to the point where she would throw her food tray across the room or, on one occasion, physically lash out at the dietary aide when she came to remove Lenore's tray.

In good agreement with Phinney et al. 10 , we have shown that dietary Cu elicits the same effects as genetic modulated Cu levels 11 . To investigate the influence of bioavailable environmental Cu, transgenic APP mice received an oral treatment with Cu-supplemented sucrose-sweetened drinking water. Chronic overexpression per se reduced superoxide dismutase-1 (SOD-1) activity in trans-genic mouse brain, which could be restored to normal levels after Cu treatment. A significant increase of brain Cu indicated its bioavailability on Cu treatment In the second study Sparks and Schreurs 15 challenged rabbits with a high-cholesterol diet and reported that the intake of minor concentrations of Cu (0.12 mg L) in the drinking water impaired the behavioral phenotype. Both intraneuronal and extracellular (neuropil deposits) Ap levels were elevated. Unfortunately, no brain Cu levels were shown.

Nutrient partitioning between fat and muscle

Adipose tissue hypertrophy is enhanced when fatty acids and glucose are preferentially channeled to adipose tissue rather than to other tissues, and particularly to muscle, where their main metabolic fate is oxidation. Two important players in nutrient partitioning between fat and muscle are LPL and GLUT4, both of which are highly expressed in the two tissues. Disregulation or imbalances of these two activities in muscle and adipose tissue may contribute to obesity, or may even cause it. Muscle LPL activity is inversely correlated with percentage body fat and body mass index in humans,60,61 and moderate overexpression of LPL selectively in skeletal muscle prevents the development of diet-induced obesity in transgenic mice.62 In mice, transgenic overexpression of GLUT4 in adipose tissue results in an obese phenotype,63,64 whereas lack of GLUT4 in adipose tissue (through tissue-specific knockout) results in reduced adiposity.65 translational mechanism,66 and recovers upon refeeding.

The Geriatric Nutrition Risk Index (GNRI) is an adaptation to the Nutritional Risk Index (NRI). The GNRI is specifically designed to predict the risk of morbidity and mortality in hospitalized elderly patients (Table 1.7) 38 .
